For Medicare claims, it's mandatory., that you enter modifiers on each and every cpt code, there's a modifier (AT or GA as mod1, depending on whether it's an active treatment or maintenance and GY as mod1 for sure, and maybe 25 or 59 as mod2 depending on what type of a code it is). If you miss out on presetting these modifiers on any of the cpt codes used, then the system doesn't allow you to code charge. To preset modifiers, go to admin > ins. carriers > find the payor > modifiers/ allowed amounts > add.
